Prior to Kim Cole's coronation to the office of Sheriff, the Sheriff's office has decided to ask a bunch of questions invading my personal privacy to be considered for getting FOIA responses at a discount, even though I have presented an affidavit myself earlier that I swore under oath before a notary that I qualified for this benefit, and accept any penalty for fraud if that weren't the case. They can accept my form, or accept this one on their new form with redacted entries (due to an invasion of my personal privacy) which I make available here.

Here is my affidavit of indigency, before signing  and notarization, which covers the three basics of what legally categorizes oneself as indigent.  Statutorily, this is what I need to provide.  You will note that the County did decide to mimic my basic form at the top of it:
